• Gender
• DOB  & Age as on 1st Jan 2016
• State, District, Village, Place of Birth
• Father's/Mother's/Husband's Name
• Place of Present Ordinary Residence
• Details of the Constituency
• Your Photograph
• Identity Proof
• Address Proof

Step- 3 Fill Other Details and Submit

Once you submit, your online application will be considered complete. Now take the further action as instructed.
